Clearly observable objective service design errors will be narrated here.
Usually “bad practices” needs some further analysis as there will be people still defending them as the only way to conduct their business, or the best of a number of bad options to chose from.
However, sometimes, the design errors can be seen at first sight. There will always be explanations for why things were not designed in a better way and we are not in the business of criticizing past decision. But they are certainly a good opportunity to learn from.
